The goal of this project is to apply ICT in order to provide an innovative solution for truly integrated patient-centric care for CHF.

This solution will comprise several innovative components:

  • Use of ontologies and automatic inference on formalized medical knowledge of the disease domain, in order to provide advanced decision support on the workstation (so-called semantic reasoning).
  • Use of the latest telematics technology to send patient data directly from the home environment to the care providers (telemonitoring)
  • Use of new sensing technology to measure, apart from weight and BP, also more advanced signals such as electrocardiogram (ECG), electrical impedance tomography (EIT), activity, etc.
  • Multimodal communication between the actors according to the latest or upcoming standards.
  • Advanced signal processing and -classification on the incoming patient data, such as the ECG.
  • Proving the possibility to make a self-learning system that can derive new rules from the history of in- and outputs. This would be the true realization of "evidence based medicine", something which has been hitherto a dream far from concrete.
